Blessé au front, lors de la Première Guerre mondiale, le jeune lieutenant américain Frédéric Henry est transporté dans un hôpital de Milan. Il trouve un grand réconfort auprès de Catherine Barkley, une infirmière anglaise, dont il tombe amoureux. Alors que Catherine vient de lui annoncer qu'elle est enceinte, l'infirmière en chef surprend leur idylle et Frédéric repart pour le front.
